1. Introduction
By booking and attending treatments at Base Physiotherapy & Pilates you agree to comply with the following Terms and Conditions. These terms are designed to ensure a smooth, professional, and safe experience for all clients, staff, and practitioners.
2. Appointments
· Booking Appointments: All appointments must be scheduled in advance through our clinic’s booking system (Pogozo app, phone, email, or online). Clients are advised to book appointments as early as possible to secure their preferred time.
· Cancellations and Rescheduling: We require at least 24 hours' notice for cancellations or rescheduling of appointments and classes. Late cancellations and no-shows without prior notice will be charged the full session fee.
· Late Arrivals: If you arrive late for an appointment, the session will end at the originally scheduled time, and the full fee will still apply. To ensure the best experience, we advise arriving at least 5 minutes before your scheduled time.
3. Payment and Fees
· Payment Methods: We accept payments via credit/debit cards, EFTPOS, cash (the exact amount as we do not have change), and other methods as outlined at the clinic. Payment is due at the time of service unless otherwise agreed.
· Pricing: The cost for treatments will vary based on the service provided. The current pricing schedule can be found on our website or is available from the clinic staff. Prices may change periodically, and you will be notified in advance of any changes.
· Package Deals and Discounts: Any discounts or package deals (e.g., bulk sessions) are only valid when purchased upfront. These are non-refundable and are to be used within the specified time frame.
4. Clinic Policies
· Health and Safety: Our clinic follows strict health and safety protocols, including cleaning and disinfecting equipment and spaces between appointments. Please inform us of any health conditions, injuries, or changes in your physical condition prior to your session to ensure appropriate treatment.
· Client Responsibilities: Clients are responsible for following the instructions provided by the physiotherapist or Pilates instructor, including any home exercise plans. It is important to notify your therapist or instructor if you experience any pain or discomfort during the session.
· Consent to Treatment: By attending your scheduled appointment, you acknowledge and provide consent to the treatment plan discussed and agreed upon during your initial assessment. You may withdraw your consent at any time during the treatment.
5. Treatment Protocols
· Physiotherapy Treatment: Treatments may involve manual therapy, exercise prescription, joint mobilizations, dry needling, or other modalities. You will be informed of the nature of the treatment and any potential risks during your consultation.
· Clinical Pilates: Pilates sessions may be one-on-one or in small groups. All exercises are tailored to your individual needs. It is important that you communicate any discomfort, medical history, or injuries before starting Pilates, as the exercises are designed to address specific goals.
6. Confidentiality and Privacy
· Patient Information: All personal and medical information provided to the clinic will be kept confidential and stored securely. This information will only be shared with your consent or as required by law (e.g., for insurance claims or legal obligations).
· Treatment Records: Your treatment records will be kept for a period of time as required by law, and you have the right to request access to these records upon written request.
7. Insurance and Payments
· Health Insurance: If you are claiming through private health insurance, please inform us prior to your appointment. It is your responsibility to check with your insurance provider about your coverage and any limits on your policy.
· Medicare: If you are eligible for Medicare rebates, please bring your relevant documents to your appointment. We will assist in submitting claims as required by the program, but it is your responsibility to confirm eligibility and coverage.
8. Treatment Risks
· Acknowledgment of Risks: Physiotherapy and Pilates treatment, like any healthcare service, carries inherent risks. These include, but are not limited to, temporary soreness, discomfort, or aggravation of pre-existing conditions. Every effort is made to minimize risks, and all treatments will be discussed with you prior to commencement.
· Discomfort During Sessions: If you experience any pain or discomfort during a session, please inform your physiotherapist or Pilates instructor immediately. They will adjust your treatment or modify the exercises to suit your needs.
9. Limitations of Liability
Limitation of Clinic Liability: Base Physiotherapy & Pilates will take all reasonable steps to provide a high standard of care, but we cannot accept liability for any loss, injury, or damage sustained during or after treatment, unless directly attributable to the negligence of clinic staff.
Client Responsibility: You acknowledge that you are responsible for your actions, choices, and the decision to participate in any exercise or therapy program. It is essential to follow the instructions of your therapist to minimize the risk of injury.
10. Termination of Services
Base Physiotherapy & Pilates reserves the right to terminate or refuse treatment in cases of:
· 10a. Non-compliance with treatment plans or policies.
· 10b. Verbal or physical abuse towards staff.
· 10c. Failure to attend multiple appointments without reasonable notice.
11. Changes to Terms and Conditions
We reserve the right to update or modify these Terms and Conditions at any time. Changes will be communicated via email or posted on our website. By continuing to book or attend treatments, you accept the revised terms.
